What do you print on a scrap tag so the next lot does not repeat it?

Scrap is going into a red bin with a marker scribble. A week later nobody can say if it was a bad offset, a bad casting, or a tool. The same bracket has been scrap three times.

One line for the part and revision, one line for the dimension that failed and the reading, one line for the cause you actually know. If you do not know the cause, write that, do not invent a story.

Keep the tag on the part, not on the bin. Bins get emptied into one pile.

If the same dimension fails twice, the setup sheet is wrong or the tool life number is a wish. Change one of those before the next lot, and note the change on the tag you keep.
