Mixed pull studs are a dropped-tool story. The changer is set for one stud length and one knob angle. Sort the crib so this machine sees one stud specification, and replace any stud with a mashed shoulder.

A drop at the end of a heavy program can also be a retention knob that loosened under vibration. Torque them. A fork that 'looks straight' still needs the arm height checked against a holder in the spindle, not against a photo.
