A pip means the center is not in a clean cone, or the cone was not deburred and the center sits on the burr. Recut or clean the center drill so the live center seats on the cone angle, then bring the tailstock up again and indicate the shaft at both ends before the finish.

Taper big at the tailstock is also a tailstock that is low or off to one side. Indicate a test bar between centers before you wear-offset the finish tool to chase it.
