A mill-turn Okuma has a milling pass along the part that clears the main chuck and clips the subspindle tooling that was parked closer than the CAM knew. The OSP simulation did not have that live tool in the subspindle.

Put every parked tool and the subspindle position into the interference picture, or index them to a known safe park and lock that park in the program. A milling pass that clears the main chuck can still cross the sub.

Run the milling approach with the subspindle sent home the first time, in single block, before you trust a simulation that was missing hardware.
